The Skulls item is a quest collectible tied to the hidden Escape Route quest, which is discovered within The Lich King quest by jumping into the well in the center of the ritual site. Completing it requires collecting 20 skulls.
Like Bones, skulls have value and can sometimes be purchased from merchants. Purchased skulls still count toward the quest, allowing clever players to prepare in advance before entering the arena dungeon.
